Assumption of Risk

I understand that interacting with and training dogs involves inherent risks, including but not limited to bites, scratches, punctures, falls, injury to myself, my dog, other people, or other animals, and damage to property. I voluntarily and knowingly accept all such risks associated with my participation and my dog's participation in any KC Canine program, session, or evaluation.

Release & Hold Harmless

I, for myself and on behalf of my heirs, assigns, and personal representatives, hereby release, waive, discharge, indemnify, and hold harmless KC Canine, Kevin Czyzewski, and all agents, employees, volunteers, contractors, and assigns from any and all claims, demands, damages, causes of action, or liability of any kind arising out of or related to my dog's training, behavior, transport, or participation in any program, including any injury to person or property, whether caused by negligence or otherwise.

No Guarantee of Results

I understand that training results are not guaranteed and depend on the dog's temperament, history, health, the handler's commitment, consistency, and follow-through. KC Canine provides structure, guidance, and methodology — not a warranty or guarantee of any specific behavioral outcome or resolution.

Medical Emergency Authorization

I authorize KC Canine to seek emergency veterinary or medical care for my dog or myself in the event of injury, illness, or emergency during a training session, at my sole expense. I agree to be financially responsible for all veterinary and medical costs incurred.

Vaccination & Health Certification

I certify that my dog is current on all required vaccinations (including rabies, distemper, and bordetella as applicable) and is in good health. I will notify KC Canine in writing of any medical conditions, medications, injuries, or physical limitations prior to training. I will keep vaccinations current for the duration of any program.

Behavioral Disclosure

I have fully disclosed all known behavioral issues, including any history of aggression, biting, resource guarding, fear reactivity, or triggers. I understand that withholding information about my dog's behavior creates a safety risk to handlers, other dogs, and the public, and may result in immediate termination of services without refund.

Owner Responsibility & Control

I understand that I remain the owner and legal custodian of my dog at all times. I am responsible for my dog's behavior outside of training sessions and for maintaining the training through continued practice. I agree to follow all handler instructions and safety protocols provided by KC Canine.

Photo & Video Release (optional)

I grant KC Canine permission to photograph and record my dog and me during training, and to use such images and video for training, educational, promotional, and marketing purposes without further compensation. I may opt out of this release without affecting my training.
